On the YubiRadius 3.6.0 appliance requests are passed internally by a web request.
If the password that is passed in the request includes a # or & symbol, the url is truncated, and the rest of the password, and otp are not received.
I don't currently have a work around for this, except to have my users change their passwords.

This is a known bug and it is fixed in version 3.6.1.

Version 3.6.1 will be released soon to the public.
