I try setting up openssl on OS X 10.11 with a hardware key via PIV/PKCS11. I inserted a new NEO and tried:
$yubico-piv-tool -s 9a -a generate
Failed to generate new key (error 6982)

$yubico-piv-tool -a version
Application version 0.1.2 found

Yubikey firmware version 3.3.0

What could be the reason?
