Over the last couple of days I've started getting authentication failures with the codes that it is producing. After a few tries it usually works but it's taking more and more tries.

What is going on here?
What can I do to fix it?

Steps to reproduce
1. Try to log in with site set up for 2FA with Yubico Authenticator
2. Tap YubiKey Neo on phone
3.
