Assuming you're referring to TOTP and not HOTP, the only realistic explanation is the system time on the phone is off.

I have been using the Android Yubico Authenticator with a YubiKey Neo without problems for about a year now. Over the last couple of days I've started getting authentication failures with the codes that it is producing. After a few tries it usually works but it's taking more and more tries.

What is going on here?
What can I do to fix it?

Steps to reproduce
1. Try to log in with site set up for 2FA with Yubico Authenticator
2. Tap YubiKey Neo on phone
3. Type produced code for the given site in

Expected result Successful login
Actual result Login failure

Yubico Authenticator for Android version 1.1.0
Samsung Galaxy S6 running Android 6.0.1
