This is related to the YubiApp on iOS, not a Yubikey.  I'm not using the YubiCloud, I'm using YubiRADIUS.<br /><br />I have tired to validate multiple OTPs generated by this YubiApp key and they all fail.  My guess is that some how the YubiApp's session counter was reset and it is therefor generating OTPs that are previous to the last entry in the YubiRADIUS DB.  The APP has not been restored or reinstalled, nor has the phone.<br /><br />I've resolved the issue by generating a new key. Hello GregL,<br /><br />What model and firmware version is your Yubikey? Did you tried to validate 5-10 OTP and always got the same error? I'm using the YubiApp on my iOS device.  Recently it stopped working, and digging into the auth logs on the YubiRADIUS and testing showed it was returning REPLAYED_OTP errors.
