SOLVED: found my answer on sourceforge/keepass/google authenticate discussion HERE. Changed my "look ahead" to 9 as it allows 3 failed attempts before yubi and keepass out of sync. It worked so that confirms I got my yubi outputs ahead of keepass and caused the errors

So I will reconfigure slot2 and start over with keepass so as to lower my "look ahead" count and try again.

Yubikey Neo firmware 3.4. Keepass 2.30 w/OtpKeyProv plugin.

Set config slot 2 as instructed got error, cleaned slot 2 started over reconfigured master key in KeePass. Keep getting this error:
