[Question]10.12.6 OSX: Yubikey credential window hangs

Trying to install software with Yubikey[Applet Version: 4.3.3 ; Version 1.4.1 ; Library versions ykpiv : 1.4.2] on OSX 10.12.6 and noticed that after entering the authentication PIN, Yubikey credential window is stuck(greyed out). Hitting the "return" key doesn't help.

Want to troubleshoot the issue and was looking for some pointers on which location to look for Yubikey logging the authentication events and any troubleshooting tips related to Yubikey logging is appreciated.

Syslogs and /var/log/asl/logs are of no help.
