trying to convert my YubiKey NEO to OTP+CCID using the YubiKey NEO Manager
v1.4.0. When I select the CCID check box I get the following error:

Failed setting the mode. If you have an Access Code protecting either of the
YubiKey slots, you will need to disable this before you can change the mode.
I'm not aware of any Access Code that I have set up. I have followed the
instructions for installing ifd-yubico.pkg as outlined in the post:
viewtopic.php?f=26&t=2049.
I reinstalled YubiKey NEO Manager after applying the above update. However I
still get the same error.
I also get the following error when trying to use ykneomgr.
